Federal Stafford Student Loan Forms
The MPN and Entrance counseling are required for any student who is a first time borrower of Federal Stafford (Student) Loan. These must be completed prior to receiving these funds.  The Exit Counseling is required upon graduating or dropping below half-time enrollment.

  • Federal Stafford Lender list and Master Promissory Note (MPN)
    New Students must complete a Master Promissory Note prior to receiving loan funds.  Students can complete this on-line with one of the lenders listed in this link.  If you choose one of these lenders, your funds will be disbursed electronically to your student account.  This provides quicker receipt of funds.
  • Stafford Entrance Loan Counseling
    This interview process provides valuable information about the Federal Stafford Loan Program. It is a Federal requirement that recipients must complete this step prior to receiving funds from this program.
  • Stafford Exit Loan Counseling
    Federal regulations require all students who borrowed Stafford funds to complete an exit counseling when graduating or dropping below 1/2 time enrollment.  This must be completed even if you intend to re-enroll the next semester.  This is required for graduation and must be completed before a diploma will be issued.
  • Stafford and Perkins Combined Exit Loan Counseling
    For those students who have borrowed both Stafford and Perkins loans you may complete this combined exit counseling. Completion of this session will satisfy Federal regulations for both Stafford and Perkins loans. All students who borrowed Stafford and Perkins funds must complete exit counseling when graduating or dropping below 1/2 time enrollment. This must be completed even if you intend to re-enroll the next semester. This is required for graduation and must be completed before a diploma will be issued.

Federal Perkins Student Loan Forms
The Entrance Counseling form is only required for first time borrowers of a Federal Perkins Loan. The Exit Counseling form is required upon graduating or dropping below half-time enrollment.

  • Perkins Entrance Loan Counseling (Need Adobe Acrobat)
    This is required for any student who is a first time borrower of a Federal Perkins Loan. This must be completed prior to receiving these funds. You will complete this on-line by clicking Student Loan Counseling Interview and Perkins Entrance.
  • Perkins Exit Loan Counseling
    Federal regulations require all students who borrowed Perkins funds to complete an exit counseling when graduating or dropping below 1/2 time enrollment.  This is required for graduation and must be completed before a diploma will be issued.
  • Perkins and Stafford Combined Exit Loan Counseling
    For those students who have borrowed both Stafford and Perkins loans you may complete this combined exit counseling. Federal PLUS (Parent) Loan Forms
These forms must be complete by the parent borrower.

  • Federal PLUS Lender List and Master Promissory Note (MPN)
    New Parent borrowers must complete a Master Promissory Note prior to receiving loan funds.  Parents can complete this on-line with one of the lenders listed in this link.  If you choose one of these lenders, the funds will be disbursed electronically to your student's account. This provides quicker receipt of funds.

Alternative Loan Forms
This form is only required for student borrowers of an alternative (non federal) loan.

  • Alternative Loan Lender List and Promissory Note
    Students must complete a Promissory Note prior to receiving loan funds.  Students can complete this on-line with one of the primary lenders listed in this link.  If you choose one of these lenders, your funds will be disbursed electronically to your student account.  This provides quicker receipt of funds.
